Yes! These forms must be filled out before we can schedule the patient. 

Without the forms, we cannot create the patient in our charting system, and are physically unable to schedule an appointment for them. 

These documents are also necessary to our medical assistants, so we may prepare for your visit and better assist you. 

Even if you have a referral, and even if another doctor's office calls us, we need this paperwork! 

If there is a genuine emergency, we recommend the Emergency Room or Urgent Care Facilities. 

Based on the symptoms and possible severity of the patient's condition, the medical assistants and Dr. Sabet confer to find the best appointment slot for the patient. 

Dr. Sabet has a high volume of patients, and he works diligently to make sure everyone has a chance to be assisted. 

If the scheduled appointment does not suit your needs, let staff know, and we will put you on the cancellation wait-list. 

If the scheduled appointment feels inappropriate due to a change/worsening in symptoms, please call our staff. We will re-consult the appointment time decision; we may move your appointment, or we may recommend staying in the current appointment slot, or we may recommend Urgent Care Facilities.
