About New Patient Registration
Dr. Sabet is currently accepting new patients for neuro-ophthalmological conditions.
New patients are required to fill out the New Patient Registration Forms. These documents provide us with essential medical information, which allows us to better aide in your health and address your concerns. The form can be downloaded below, mailed to you, or picked up from our office in person.
New Patient Registration Forms must be returned to us before we can schedule you for an appointment. Please also include a copy of your insurance cards!
Forms can be returned to the office by fax, mail, or dropping them off in person. Unfortunately, we are not able to receive these forms by email in order to uphold patient information security.
