About New Patient Registration
Dr. Sabet is currently accepting new patients for neuro-ophthalmological conditions.
New patients are required to complete and return the New Patient Registration Forms before an appointment can be scheduled. These forms provide us with essential medical information that allows us to better understand your medical history and address your concerns.
There are several ways to complete the forms:
- Online: If you prefer to complete and submit the forms online, please call our office and we will provide you with a link to the online forms.
- PDF: You may download and print the PDF forms below, complete them, and return them to our office.
- In person: Forms may also be picked up from our office.
Once completed, forms may be returned by fax, mail, or by dropping them off in person. Unfortunately, we are unable to accept completed forms by email in order to protect the security and privacy of your medical information.
Please note: We must receive the completed New Patient Registration Forms before we can schedule a new patient appointment. Please also include a copy of your insurance card(s).
